The purpose of the Employee Evaluation Form is to assess a person’s performance and ongoing development. The form can be used during ongoing employee reviews, as part of the appraisal process, or to track an employee’s personal development and  identify any training needs required to close competency gaps.
The form include to key sections beside the employee information. Forst section is core values which include elements such as quality of work, attendance & punctuality and communication skills amongst other. The second section is related to job specific performance.
